As part of Legacy Law Center's passion of taking care of families, our team includes an Elder Care Coordinator who oversees the coordination of your loved one's health and long-term care, serves as a resource and advocate for your family, and empowers you with the information and knowledge to help you make the right decisions for your loved one in many circumstances. Our Elder Care Coordinator has extensive knowledge of available community resources to assist your loved one whether they are living at home, at an assisted living facility, or in a skilled nursing facility. Our Elder Care Coordinator can help provide valuable information to you on costs, quality, and what you can expect in your loved one's care.
What Does an Elder Care Coordinator Do?
Your Elder Care Coordinator serves you in many ways during the process of care coordination for your loved one. Your Elder Care Coordinator functions as your advocate, your support, and your friend. Examples of some of the tools/actions that your Elder Care Coordinator will take in working with you and your loved ones include:
- Conduct care assessments to uncover potential problems and to help find solutions
- Help you identify and hire in-home help or services from community resources
- Coordinate with medical and healthcare providers
- Help with financial bill paying and budgeting of financial resources
- Review medical issues and offer referrals to geriatric specialists
- Provide support, guidance, and advocacy in a time of crisis
- Help to coordinate the transfer and transportation to medical appointment
- Provide education, counseling, and support
- Arrange for alternate housing, if needed
- Assist with Medicaid and VA Benefits planning
- Work in collaboration with other member's of your loved one's support team to insure that each aspect of your loved one's needs are being met appropriately
